The Speech/Language Pathologist (SLP) evaluates, develops, implements and documents an intervention plan. This position provides intervention, evaluation and documentation of progress toward goals. The SLP develops and documents a discharge plan, completes all documentation required by multiple service sites or facilities, and meets requirements of third party payers.

ESSENTIAL JOB FUNCTIONS
  1. Supports and models behaviors consistent with the mission, vision and values of Altru Health System.
  2. Performs pain assessment and takes appropriate action according to standard operating procedure.
  3. Evaluates, develops, implements and documents an intervention plan using methods that are effective, within accepted treatment guidelines and are meaningful to the patient's plan of care through remediation/restoration, compensation/adaptation, disability prevention and health promotion.
  4. Provides intervention, evaluating and documenting progress towards goals, modifying intervention methods as needed, working with interdisciplinary therapy team members to maintain patient's goals as primary goals.
  5. Develops and documents a discharge plan when patient has reached goals, achieved maximum benefit from therapy or does not desire to continue service. Discharge plan addresses appropriate follow-up resources and services and training of patient and family members as appropriate.
  6. Completes all documentation according to standard procedures on time and within practice guidelines.
  7. Completes all documentation required by SNFs, contracted outpatient sites, and Altru outpatient sites upon evaluation, throughout treatment progress, and upon discharge.
  8. Demonstrates the ability to use multiple EMR systems including but not limited to Altru EPIC, Sanford EPIC, Rehab Optima, Point Click Care and American Health Tech.
  9. Demonstrates a working knowledge of Medicare A and Medicare B guidelines as related to G-codes, RUG levels, and documentation guidelines. Effectively communicates therapy goals and treatment timelines with PT/OT staff as well as pertinent contact site staff in order to ensure therapy RUG levels are achieved and Medicare guidelines are followed.
  10. Meets requirements of third party payers by including appropriate documentation following window periods and other insurance guidelines.
  11. Must be able to balance schedules at multiple sites daily and adjust schedules quickly to accommodate changes or new patient evaluations as necessary.
  12. Develops and maintain relationships with key facility staff at all sites including, but not limited to, the administrator, director of nursing, MDS coordinator, and dietary manager as well as the restorative nursing staff.
  13. Performs other duties as assigned or needed to meet the needs of the department/organization.

KNOWLEDGE & SKILLS
Required:
  • Master's Degree in Speech/Language Pathology.
  • Licensed by the state of North Dakota in Speech/Language Pathology and/or licensed by Minnesota in Speech/Language Pathology.
  • Certified by the American Speech Hearing Association (ASHA) with CCC-Sp or CFY with a qualified Speech/Language Pathology clinical supervisor and will obtain ASHA CCC-Sp within three (3) years from date of hire.
  • Must have a valid Driver's License and use of a personal vehicle.
  • Successful completion of the functional assessment.

  • Previous experience.
