JOB SUMMARY

The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in assisted living, under the supervision of a registered nurse, advanced practice provider, or physician.
Demonstrate knowledge of nursing principles for the resident in the relevant clinical setting. The LPN demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards related to the physical and psychosocial needs of the resident as per care plan. The LPN must possess the ability to perform testing to evaluate data reflective of the resident's status and to provide the care needed. Demonstrates ability to recognize problems, recommend solutions and respond effectively to assist in an emergent situation.
Ensures the health, comfort and safety of residents. Contributes to the assessment of residents and administers medications or treatments as ordered. Provides technical support to healthcare professionals as needed. Documents resident interactions as needed and the outcomes related to resident care, medication, behavior, activity, and dietary. Skilled in establishing and maintaining effective working relationships with resident, health care providers, and the public. Ability to work effectively with professional and supportive personnel as a member of an interdisciplinary care team. Ability to communicate clearly and maintain confidentiality of sensitive information. Counsels residents, family members and/or resident advocate, under the direction of a registered nurse, advanced practice provider or physician on prevention and treatment plans. Knowledge of resident education principles to develop a self-care program. Performs other related duties as assigned.

POSITION RESPONSIBILITIES

Responsible for promoting a safe environment for patients, families, staff, residents, and visitors and working collaboratively with the multi-disciplinary health care team to provide the best outcomes for patients. Promote a safe and therapeutic environment by providing appropriate monitoring and surveillance of the care environment. Participate in health teaching developed by a licensed practitioner or registered nurse. Participate and be part of a healthcare team to conduct focused nursing assessments and contribute to the plan of care. Observe patients, document and report changes in patient condition, such as adverse reactions to medication or treatment, and take any necessary action. Implement interventions that are assigned, ordered or prescribed by licensed practitioner or registered nurse. Follows safety measures to ensure proper use of equipment and supplies. Establishes and maintains productive working relationships within and outside of own area. Effectively delegates and supervises based on one's state practice act and an understanding of the concepts of responsibility, authority, and accountability. Contribute to the evaluation of the plan of care by gathering, observing, recording and communication patient responses to nursing interventions. Communicate relevant information with patients and health team members to ensure quality and continuity of care. Adheres to legal regulations and organizational policies when discussing patient health information.

DEPARTMENT DETAILS

Full time; 64 hours every 2 week pay-period.

QUALIFICATIONS

Graduate from an accredited practical nursing program as a Licensed Practical Nurse. Associate degree in practical nursing is preferred.
If position requires IV therapy/management skills, the following is required in North Dakota: graduation from a Board approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing.
One year of experience preferred in one of the following settings: long-term care, sub-acute care, or medical/surgical. Specific services/positions may have additional education, training, or experience requirements.
Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges. Obtains and subsequently maintains required certifications and completes required continuing education units (CEU) and competency training.
