Scope
• Primary focus is on the individual needs of those seeking help within our continuum. The navigator helps guide the patient through the healthcare system and works to overcome obstacles that are in the way of the patient, client or resident receiving the care and treatment they require.
• Expert for all Knute Nelson divisions and services.
• Serves as the point of contact and resource for all Knute Nelson services for providers, patients, family and facilities.
• Uses clinical expertise and training to guide patients and their caregivers to make informed decisions, collaborating with a multidisciplinary team.
• Serves in a consultant role, acting as an advocate and marketer of all Knute Nelson services to hospitals, clinical settings and community partners.
• Navigates patients, residents and clients through the Knute Nelson environment. Helps evaluate processes to ensure we have a seamless referral and admission process.
• Collaborates closely with our internal teams to determine eligibility of individuals for services.
• Collaborates closely with our internal teams to provide ongoing interdisciplinary education to ensure you have the best team of advocates supporting our mission. • Interacts and facilitates positive relationship development among referral sources and community partners.
• Conduct visits to Alomere Hospital for Knute Nelson patients, clients or residents for hospitalizations, re-hospitalizations and/or simply for new patients interested in learning more about Knute Nelson services and how we can help one another.
• Lead and participate in developing and enhancing tools and educational programs that promote Knute Nelson services.
• Supports and participates in marketing engagements related to community engagement and building the presence of Knute Nelson in the community

Key Responsibilities
• Navigator Duties- Works closely with referring providers, primary providers, and Knute team, regarding criteria for services. Collaborates with facility Case Managers to coordinate care. Attends care conferences as needed. Interacts and facilitates positive relationship development and serves as liaison among community partners. Is accessible via phone and email to providers, peers, and Knute team during working hours. Represents the face of Knute Nelson for residents, patients or clients admitted to the hospital, care center or other facilities. Flexibility in work schedule to accommodate needs of patient, family, and facilities. Facilitate communication of patient status and plan of care during transitional experiences. Collaborates with divisions to ensure strong collaboration internally. Responsible for patient, client and resident visits and care coordination when admitted to a hospital or care center. Participates in developing and enhancing tools and educational programs that promote Knute services. Manages time effectively to ensure all duties and documentation requirements are completed promptly. Maintains communication with the division leaders regarding compliance, job performance, and significant patient care and facility issues as they arise.
• Growing and Maintaining Market Share Responsible for growing market share and geographic territory for Knute Nelson services. Success will be measured by total monthly referrals, monthly conversion ratio of referrals to admissions, total monthly events, total monthly census, and other measurements as determined.
• Interpersonal Communication/Problem Solving - Adopts communication styles appropriate to the listeners and situation. Listens actively, asks questions, clarifies points and rephrases others’ statements to check mutual understanding. Modifies communication in response to feedback from listeners. Works effectively with others to resolve conflict in a constructive way that builds relationships and drives results.
• Customer First Attitude - Makes the customer experience a primary focus of all actions. Understands that the customer extends beyond the patient and family, including community partners and referral sources, etc. Consistently develops and sustains mutually beneficial working relationships. Is accessible and provides prompt resolution to concerns or problems. Encourages and facilities cooperation within the organization and with customer groups.
• Accountability/Leadership - Practices accountability to daily work tasks, expectations and team norms. Identifies what needs to be done and proactively takes action to accomplish objectives and achieve company goals and priorities. Reports to: Vice President of Marketing

Education and Experience Required
• LPN or RN required • 2+ years of healthcare experience
• customer service experience preferred
• Must complete and pass a state(s) required background study.
• Must have adequate computer skills.
• Able to physically perform the duties required by the position.
• Required to have the ability to read, write and speak English well enough to communicate with staff, residents and visitors.
