Job Details
The Clinical Social Worker (LCSW) provides evaluative and mental health services to the patients/family of the inpatient unit. This position conducts patient/family assessments, short-term therapy, group therapy sessions, and referrals. Maintains knowledge of psychiatric medications and identifies significant changes or high-risk situations.
Essential Job Functions
Title and Description
Completes social history, initial and ongoing assessments of patients, develops treatment plan and reviews assessment information and treatment plan with each patient.
Provides individual, couple, family or group therapy using a variety of therapeutic approaches.
Communicates with patient, families, agencies and other appropriate individuals regarding the results of screenings, assessments, recommendations, plan of care and other information deemed necessary to facilitate access to care.
Identifies professional and organizational issues and trends, suggesting action and facilitating change and participates in restructuring of processes and services.
Makes contact with referral sources, providers and other caregivers and assures communication and coordination exists regarding patient care. Collaborates with community resources to deliver services that promote quality outcomes and continuity of care.
Makes contact with the courts and assures communication and documentation exists regarding committals, emergency holds, guardianship and tribal issues.
Identifies, conducts follow-up and takes action on unsatisfactory outcomes, unusual findings, special needs and requests.
Ensures that intervention, treatment or referrals are made according to standard operating procedure.
Performs other duties as assigned or needed to meet the needs of the department/organization.
Demonstrates understanding and follows infection control policies and procedures according to standard operating procedure. Maintains requirements for age specific competencies for the position and demonstrates the knowledge or principles of growth and developments and the skills necessary to provide patient/customer care/services.
