Job Details

The RN Patient Care Coordinator's (PCC) focus is on the day to day coordination of the unit. The PCC works to help provide adequate staffing while acting as the first resource to staff. The PCC works closely with the interdisciplinary team to help manage each patient's stay. This position is also responsible for rounding with physicians and helping to keep the lines of communication open.
  • Performs daily rounds on all patients assuring quality customer service by coordinating and taking action on any requirements not met.
  • Assigns or delegates, within the scope of practice, education and experience, the responsibility for performance of nursing interventions to other persons for which that person has the necessary skills and competence to accomplish safely.
  • As early as possible, begins discharge planning, appropriate education and counseling based on identified needs to optimize patient/family understanding. Evaluates and documents according to standard operating procedure.
  • Communicates with referring facilities or agencies regarding patient's care. Monitors patient outcome measurements.
  • Identifies, conducts follow-up and takes action on unsatisfactory outcomes, unusual findings, special needs and requests.
  • Collaborates with team, referral sources and family members to obtain a holistic view of the patient through assessment of the physical, mental and emotional status. Determines the post-hospital needs and collaborating with physicians and members of the multidisciplinary team.
  • Communicates with staff and Nursing Administration regarding bed availability and staffing needs.
  • Performs pain assessment and takes appropriate action according to standard operating procedure.
  • Performs other duties as assigned or needed to meet the needs of the department/organization.

KNOWLEDGE & SKILLS
Required:
  • Registered Nurse licensed by the North Dakota Board of Nursing or licensed through the "Compact State Agreement."
  • Must obtain Minnesota license from the Minnesota Board of Nursing within eight (8) weeks from the first day of work. (Cannot practice care in Minnesota until licensed which includes phone calls.)
  • A minimum of three (3) years of clinical experience in an inpatient setting.
  • Demonstrates the ability to effective communicate both verbally and in written format.
  • Successful completion of the functional assessment.
Preferred:
  • Bachelor's Degree in Nursing.
  • Previous supervisory experience.
  • Previous critical care and/or cardiac experience.
  • Demonstrates the ability to use various software programs.

  • Demonstrates understanding and follows infection control policies and procedures according to standard operating procedure.
  • Maintains requirements for age specific competencies for the position and demonstrates the knowledge or principles of growth and development and the skills necessary to provide patient/customer care/services.
  • Demonstrates knowledge and understanding of and compliance with:
    • All pertinent safety, health and environmental policies, procedures and guidelines.
    • Working safely, without causing harm or risk to self, others or property.
    • The work environment safety procedures and prompt reporting of unsafe practices, procedures, accidents, injuries or other safety violations to the Leader/Manager.
    • The use of all safety equipment and personal protective equipment.
