The purpose of this position is to support clients with every aspect of their life to include health and personal cares, therapeutic, educational, recreational, and leisure activities for individuals with special needs to achieve maximum growth and learning, as they make transitions from youth through adulthood. The incumbent is responsible to complete all necessary charting such as daily reporting, goals and objectives, annual reports, assessment reports, vocational assessments and evaluations, and teaching strategies.

This position assumes the authority, responsibility, and accountability for working with the FRD/FDC regarding individual life plans. This position supports the development and connections needed to become a vital participant in community life. The incumbent supports training in areas such as: personal cares, money management, shopping, transportation, spirituality, and recreation and leisure. The incumbent will work independently with clients while at their home or worksite and throughout planned daily activities. This position will make independent decisions regarding the well being of the client. This position can either be a full-time, part-time or on call.

We will train and certify new staff as part of the on-boarding period.
