JOB SUMMARY
Serves as case manager to assigned Home Care clients and families, assesses plans, teaches, gives and evaluates nursing care. Responsible for setting up medications doing assessments and supervising Home Health aides. Required to input information into computer system.
QUALIFICATIONS (minimum requirements)
Education and/or years experience required:
Registered Nurse degree from an accredited college. Previous experience in Home Health desired.
Licensure/Certifications required:
Current Registered Nurse licensure to practice professional nursing in the State of Minnesota.
Supervision of others/decision making aptitude:
Supervise the performance of the home health aides.
