JOB TITLE: Healthy Communities Partnership (HCPP) Wellness Assistant

III. PERSON(S) CURRENTLY OCCUPYING THE POSITION:
IV. JOB SUMMARY: Responsible for assisting the HCPP Wellness Coordinator with the office needs, entering data, developing flyers for events, assisting with events such as Foot Lake 4, 5K community walk/run, and biometric screenings, and other duties as assigned.
V. JOB RELATIONSHIPS:
A. REPORTS TO: HCPP Wellness Coordinator
B. SUPERVISES: n/a
C. INTERRELATIONSHIPS WITH: Community, Program participants, Hospital staff and Administration, Foundation staff, HCPP Advisory Committee.
VI. HOURS OF WORK: Casual position. 5 to 15 hours per week based on workload.
VII. DRESS CODE: Business casual. Follow Rice's dress code. Name badge.
VIII. ESSENTIAL REQUIREMENTS OF WORK (Minimum qualifications necessary to function at full productivity).
A. EDUCATION AND TRAINING: High School diploma. Knowledge and experience in healthcare or wellness preferred, but not required.
B. EXPERIENCE: Experience working with a diverse groups of people. Able to work with basic computer programs such as Word, Excel, Power Point, and Health Risk Assessment tools. Office experience and strong public relations skills.
C. JOB KNOWLEDGE (Specific): Knowledge of health and wellness preferred. Knowledge of community resources. Good organizational skills in order to conduct screenings in the community and track individuals that have been screened.

XI. PHYSICAL DEMANDS OF THE JOB: Walking and standing frequently, reach with hands or arms, talk, hear. The employee may frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Close and distance vision required.
XII. PHYSICAL ENVIRONMENT OF THE JOB: Office setting, community settings both indoors and outdoors.
XIII. EQUIPMENT OR MACHINES USED ON THE JOB: Biometric screening tools, computer, lap top, copy machine.

A. JUDGEMENT AND INITIATIVE: Serve as an agent of the HCPP Program under the assignment and direction of the Wellness Coordinator.
B. INDEPENDENT ACTION: Needs to exercise independent action under general direction.
C. EFFECT OF ERROR: May impact the success and sustainability of the Healthy Communities Partnership.
D. CONTACT WITH OTHERS: Regular contact with program participants, community members, and community stakeholders.
XV. PRINCIPAL JOB RESPONSIBILITIES, TASKS, AND AUTHORITIES:
A. RESPONSIBILITY: Provide support for the Wellness Program to ensure the efficient flow of information and programming within the Wellness dept and the hospital.

- Assist the Wellness Coordinator with various wellness projects.
- Work closely with both the Wellness Coordinator and Wellness Coach to be sure all needs of this dept are being met.
- Develop relations with community, schools, businesses etc. per direction of Wellness Coordinator.
- Keep appropriate information on community involvement and events.
- Report details of the events when requested.
- Assist in assembling volunteers as needed.
- Assist with any events, special projects as directed.
B. RESPONSIBILITY: Assist in coordinating and ensuring sustainability of the Healthy Communities Partnership.

- Assist in setting up and promoting the Healthy Community Partnership program.
- Promote the program to various community groups at the request of the Wellness Coordinator.
Engage with community members to promote the program as needed.
