Position Summary

Provide bereavement support to hospice families in coordination with the interdisciplinary team and provide Journey programming to the broader community related to end-of-life and grief issues.

This position can be located in the either the Bismarck area or the Devils Lake area.

Qualifications
  • Bachelors in science in Social Work, master's in counseling or related field
  • Training and experience with grief and loss counseling of individuals and families; and group facilitation
  • Training and experience in family dynamics and psychosocial assessment
  • Experience in public speaking
  • Strong communication skills
Certifications, Licenses, Registrations Required
  • Licensure in North Dakota and Minnesota (willing to obtain dual licensure upon hire)
  • Current driver's license and auto insurance
Key Responsibilities
  • Following a patient death, provide initial bereavement needs and psychosocial assessments of clients desiring bereavement services.
  • Develop a plan of care with the client and IDG members that identify the needs, outcomes, and interventions for each client.
  • Document all client contact and interventions in a timely and accurate manner.
  • Provide ongoing bereavement services and psychosocial assessments for up to 13 months post death.
  • Refer clients to support groups and community resources as appropriate.
  • Facilitate grief support groups for grief clients and the general public.
  • Maintain documentation of support group and other bereavement service for program evaluation.
  • Provide resources, referrals and community education presentations regarding grief and loss and end-of-life issues in the community.
  • Assist in the development of bereavement programs and services.
  • Report program activity and issues to Journey's manager.
  • Properly track community presentations.
  • Participate in the outcomes measurement of bereavement programs.
