STATEMENT OF PURPOSE

Under the supervision of the HIM Manager, maintains medical record integrity through the coding of diagnoses and operations using ICD-10-CM and CPT-4 classification systems; contacts physicians as necessary for clarification and/or to obtain additional information necessary to accurately code the medical record; enters information into computerized data base and/or notes the DRG payment to determine appropriate optimization; reviews medical records for completeness.

This is a full time benefitted position working 80 hours per two weeks. Days of work are Monday - Friday with variable hours. No nights or weekends.

JOB FUNCTIONS
  • Demonstrates an understanding of coding conventions; adheres to coding guidelines and refers to current literature regarding coding questions. Contacts physicians as necessary to obtain additional information.
  • Assigns appropriate codes within the Sanford Epic system.
  • Utilizes the 3M Encoder and stays proficient with this system.
  • Completes coding and specified charge ticket information for inpatient and observation discharges.
  • Completes coding and computer data entry for Home Health and Hospice patients.
  • Assists hospital staff, physicians, patients and guests with inquiries reĀ­garding medical information.
  • Performs other duties as assigned or requested.

QUALIFICATIONS/REQUIREMENTS FOR THIS POSITION: (HEALTH INFORMATION TECH II)

1. PREPARATION AND TRAINING - Current RHIT (Registered Health Information Technician).

2. WORK EXPERIENCE - No experience required if employee demonstrates a good working knowledge of ICD10CM. Two years experience in a medical record department, coding experience is preferred.

3. ANALYTICAL ABILITY - Must possess strong ability to assign the correct codes to diagnoses and procedures and determine the correct DRG.

4. INDEPENDENT JUDGEMENT - Responsible for maintaining standards of this position. Manager shall be contacted if questions arise.

5. CONTACTS WITH OTHERS - Communicates with co-workers, other hospital staff, physicians, and guests.

6. SUPERVISING THE WORK OF OTHERS - None.

7. RESPONSIBILITY FOR WELFARE OF OTHERS - Employees must realize the importance of confidentiality at all times. The identification of patient medical records must be accurate and concise.

8. MENTAL/VISUAL EFFORT - Must be able to speak and write the English language in an understandable manner. Must be able to see and hear or use prosthetics that will enable these senses to function adequately to assure that the major requirements of this position can be fully met. Must be in good general health and demonstrate emotional stability. Visual acuity and color perception necessary for performing routine procedures.

9. WORKING CONDITIONS - Participates in and complies with JRMC Safety Management Program. Maintains knowledge of and observes Standard Precautions. OSHA Job Classification: Category II position, employee does not have exposure to bloodborne pathogens.

10. PHYSICAL AND SENSORY EFFORT - This position requires frequent sitting to perform the major responsibilities.

11. PROMOTION - No formal line of promotion.
