  • Responsible for the administration of general care of the residents and patients.
  • Provide regular work direction and monitors the work of Nursing Assistants and Care Attendants on assigned shift.
  • Works closely with the patient/resident and with various members of other departments such as activities, meal services, social services, chaplain, housekeeping, and other members of the nursing department.
  • The LPN may have contact with the family of the resident/patient.
  • Must have flexibility in schedule to fulfill an on-call rotation if required and be available for on-call medical questions and/or emergencies via phone or in person as needed.

Education and Experience Required
  • Must be licensed as a Practical Nurse in the state of Minnesota.
  • Must obtain CPR certification within 6 months from date of hire.
  • Must complete and pass a state required background study.
  • Maintain own educational competency.
  • Requires strong organizational skills, an ability to work with a variety of residents/patients with differing values, backgrounds, and capabilities. This position requires an ability to work effectively with co-workers as a team and individually.
  • Require exceptional customer services skills and the ability to work effectively with changing priorities. Experience working with the elderly in long-term care, home care or assisted living settings is preferred.
  • Excellent communication (written and verbal) and critical thinking skills are required.
  • Able to physically perform the duties required by the position.
  • Required to have the ability to read, write and speak English well enough to communicate with staff, residents and visitors.
Key Responsibilities
  • Administer medications as doctor orders. Following all physician's orders, transcribe new orders on the medication record. Administer injections, oral medications, eye drops, inhalers, all medication doses given routes as ordered, accu-checks, Foley catheters, creams, lotions, etc. to the appropriate resident/patient.
  • Responsible for dressing wounds. Administer oxygen, suctioning, tube feedings, etc. as ordered by physician. Clean and stock the med cart, med room and nurses' station. Count all scheduled drugs in lock box and follows all applicable state, federal, and facility guidelines in the destruction of meds.
  • Obtain the necessary information on the resident/patient to initiate the Plan of Care for the resident per applicable facility, state and federal guidelines.
  • Monitor and provide work direction to Nursing Assistants/Registered (NA/R). May assist the NA/R with resident cares. Follow up with resident and NA/R to ensure proper care. Serve as a mentor and provides leadership and guidance in all aspects of NA/R's performance including completion of performance evaluation information.
  • Monitor the TMA in administration of all medications. Provide instruction in administering nebulizer treatments, oxygen treatments, glucose monitoring and Glucometer QA testing. Serve as a mentor and provides leadership and guidance in all aspects of the TMA's performance.
  • Understand the Resident's Plan of Care and recognizes changes in resident's condition. Observe and document any changes in resident's physical, mental or emotional status and resident's ability in Activities of Daily Living. If the resident's condition warrants, the Charge Nurse observes food and fluid intake. Observe for and document on reactions to medicine and treatments, pain, sleep patterns, vital signs, social behavior, etc. and informs the physician as warranted. Ensure charts, notes, and resident plan of care is up-to-date and complete per facility policy and applicable state and federal guidelines. Communicate with other shifts on resident changes.
  • Inform the resident about the medicines and treatments received and explains side effects and potential reactions. Respect the patient's/resident's decision to exercise his/her right to refuse the medication. Explain to resident procedures used in Activities of Daily Living such as transferring, toileting, ambulating, etc.
  • Respond immediately to "STAT" calls. Responsible for learning and knowing each Resident Plan of Care instructions for life saving measures. Administer CPR, oxygen, and other life saving measures as required. Follow the Resident Plan of Care instructions with regard to DNR (do not resuscitate), DNI (do not intubate), and/or DNT (do not transfer). Respond to resident incidents by observing and monitoring resident's condition and taking appropriate action.
  • Take responsibility for welfare and safety of resident during shift and promotes the resident's right to make personal choices to accommodate their needs. Promote the resident's right to be free from abuse, mistreatment, and neglect and reports any instance of such treatment to appropriate disciplines. Talk with and listens to resident during cares. Provide updates on the resident's condition to the family. During such times, offer support and comfort to a dying resident and his/her family.
  • Train and provide orientation to new and/or current employees as directed. Assist in special projects as needed by the Director of Nursing. Practice infection control procedures.
  • Is responsible for creating a home like environment for each person we serve.
