Schedule: Full-time | 64 hours per pay period | Monday-Friday | 8 am - 4:30 pm | Weekends as needed

Scope

• Responsible for performing skilled nursing services in accordance with the standards and practice of nursing, state and federal regulations, and Knute Nelson's policies and procedures.

• Float to all regions as needed to administer skilled nursing care to clients requiring professional nursing service. Educating the family on end of life and supporting the nurse visit needs of the hospice program.

• Ensure time management that best utilizes Knute Nelson's resources while meeting client needs.

• Assure that the highest degree of quality care, customer service, fiscal responsibility, and regulatory compliance is maintained at all times.

Education and Experience Required

• Must be licensed as a Licensed Practical Nurse in the state of Minnesota.

• Must obtain CPR certification within 6 months from date of hire.

• Must have a valid driver's license and use of an insured personal vehicle.

• Must be willing to travel to all regions as needed.

• Able to communicate verbally and in writing to the extent required by the position.

• Must complete and pass a state required background study.

• Able to physically perform the duties required by the position.

• Required to have the ability to read, write and speak English well enough to communicate with staff, clients and visitors.

Key Responsibilities

• Direct Client Care - Provide quality nursing care to patients in an environment which promotes their rights, dignity, freedom of choice, and their individuality. Perform full assessment with each visit. Observe temperature, pulse, respirations, blood pressure and weight of patients when so directed. Skin care with observation for pressure areas and sores. Knowledge of preventive measures. Report any deviation to the RN Case Manager. Follow the written Plan of Care. Communicate with and support patient and family. Observe deviations from the usual physical and mental conditions. Pick up and deliver medications, equipment and supplies as needed. Patient documentation will be completed same day as visit is completed.

• Client Records and Billing - Provide accurate and timely documentation of services rendered utilizing agency EMR. Demonstrate understanding and knowledge of all payer sources and authorization required. Complete and document supervisory visits with the LPN and/or HHA per regulations.

• Communication - Communicate effectively with clients, families, referral sources, physicians, and the interdisciplinary team. Demonstrate exceptional verbal and written communication skills.
