Key Responsibilities:
  • Clinical Practice
    • Support communities and drive resident-centered care delivery systems to define needs and facilitate solution strategies utilizing knowledge of the nursing process, theory, principles, and technology of nursing practice while sustaining compliance with company policies, as well as state and federal regulations.
  • Leadership
    • Direct, lead, coach, and evaluate the performance of all team members within the department. Effectively manage conflict, promote change and growth, foster a workplace that results in a high performing team, and inspire the development of a highly effective team of professionals.
    • Ensure continuity in approach and application of company policies and procedures while ensuring compliance with federal, state, healthcare, and local regulations.
    • Assist community Executive Director (ED) or Administrator with the onboarding and training of newly hired DHS/DON/RN Managers utilizing:
      • DHS/DON Competency Checklist
      • Ongoing support and training as needed
      • Company package and pricing training
      • Additional training as needed for the electronic health record software application(s)
    • Participate in the opening of new/managed communities as assigned.
  • The Regional Director of Clinical Services will oversee:
    • Clinical practices in designated communities to ensure high quality of resident care.
    • Stand-up morning meetings during site visits to ensure the set agenda is followed per company protocols.
    • Assessments are completed timely, accurately, and communicated properly.
    • Service agreements are created utilizing the appropriate electronic health record tool, consistent with the resident's current assessments and associated with the billing method.
    • Tasks and services are delegated only to unlicensed licensed personnel (ULP) that have been trained and demonstrated competency.
    • Incident and accident reports, medication errors, formal concerns, and other potential vulnerable adult concerns are identified, investigated properly, and appropriate corrective action was taken when applicable.
    • Infection prevention and control program and practices.
    • Education is provided timely when areas of non-compliance or concerns are identified.
    • Medication and therapy management services are provided according to policy and procedures.
    • Services provided to ensure they accurately reflect the resident's current mental, physical, and cognitive status and meet their individualized needs.
    • Assessment reports and package audits to ensure revenue is captured for actual services provided and review findings with the community leaders.
  • Assist and monitor communities for survey readiness.
    • Lead and participate in survey readiness activities.
    • Develop and maintain the community Survey Binder.
    • Provide active onsite support during a state survey.
    • Review and offer recommendations for vulnerable adult investigations and Plans of Correction (POC) as needed.
    • Review Vulnerable Adult Individualized Abuse Prevention Plans (VA/IAPP) and monitor for adherence of reporting guidelines to the Minnesota Adult Abuse Reporting Center (MAARC), Office of Health Facility Complaints (OHFC), and Adult Protective Services (APS) in Wisconsin.
    • Assist community clinical leaders in following and implementing all current policies and procedures.
    • Participate in vaccination and testing clinics as directed by supervisor.
  • Quality Assurance and Performance Improvement
    • Work collaboratively with community clinical leadership to support efforts in attaining and maintaining high clinical standards and the management of improvement projects.
    • Oversee completion of community audits and offer recommendations for the correction of areas identified as non-compliant.
    • Attend community Quality Assurance Committee meetings as needed.
    • Take a leadership role in company-wide quality reviews.
    • Update and provide ongoing communication of issues/concerns regarding quality of care and compliance to VP of Quality & Corporate Compliance.
    • Work in collaboration with other Shared Services team members to provide routine review and updating of policies, procedures, and forms.
    • Participate in and/or lead identified projects as assigned by supervisor.
    • Participate on internal and external industry committees either in a lead or participant role.
  • Servant Leadership
    • Build and maintain an atmosphere of respect and trust with team members, contract professionals, and outside providers.
    • Foster leadership in others.
    • Collaborate and communicate regularly with VP of Quality and Corporate Compliance, VP of Operations, and Directors of Operations.

Education and Experience Required:
  • Current unrestricted license as Registered Nurse in the State of Minnesota/Wisconsin (whichever is applicable).
  • Current BLS Certification.
  • A minimum of three years' work experience in an administrative/management clinical leadership position in Skilled Nursing and Assisted Living.
  • Strong knowledge of Minnesota's State regulations for Assisted Living (AL) licensure requirements, and Minnesota's State regulations for skilled nursing facilities (SNF) and CMS' federal regulations.
  • Understanding of state and federal regulations.
  • Demonstrated assessment skills.
  • Able to respond to emergency calls promptly, with sound judgement and follow-through.

  • Able to physically perform the duties required by the position.
  • Required to have the ability to read, write and speak English well enough to communicate with staff, residents and visitors.
