Schedule: Full-time | 80 hours per pay period | Monday-Friday | 8 am - 4:30 pm

Scope:
  • Responsible for organizing, developing, and directing skilled nursing services in accordance with the standards and practice of nursing, state and federal regulations, and Knute Nelson's policies and procedures.
  • Ensure time management that best utilizes Knute Nelson's resources while meeting client needs.
  • Assure that the highest degree of quality care, customer service, fiscal responsibility, and regulatory compliance is maintained at all times within the Home Health division.
Key Responsibilities:
  • Direct Client Care - Initiate preventative and rehabilitative nursing procedures as appropriate for the client's care and safety. Observe signs and symptoms and report to the physician reactions to treatments, including drugs, and changes in the client's physical or emotional condition. Teach, supervise and counsel the client and caregivers regarding nursing care needs and other related problems of the client at home.
  • Client Records and Billing - Provide accurate and timely documentation of services rendered utilizing agency EMR. Demonstrate understanding and knowledge of all payer sources and authorization required. Complete and document supervisory visits with the LPN and/or HHA per regulations.
  • Communication - Communicate effectively with clients, families, referral sources, physicians, and the interdisciplinary team. Demonstrate exceptional verbal and written communication skills.

Education and Experience Required:
  • Must be licensed as a Registered Nurse in the state of Minnesota.
  • Must obtain CPR certification within 6 months from date of hire.
  • Must have a valid driver's license and use of an insured personal vehicle.
  • Must complete and pass a state required background study.
  • Able to communicate verbally and in writing to the extent required by the position.
  • Able to physically perform the duties required by the position.
  • Required to have the ability to read, write and speak English well enough to communicate with staff, clients and visitors.
