We collaborate across disciplines to ensure our patients receive the best support for their mental and emotional well-being. As part of the team, you'll work with experienced providers, counselors, and resource coordinators in a supportive and innovative environment.

Mission for Position
To provide compassionate, patient-centered care through education, resource coordination, and collaboration, empowering patients to achieve mental and emotional well-being while enhancing care quality and access.
Duties & Responsibilities
- Provide patient-centered care by educating patients on medication use, symptom monitoring, and treatment plans.
- Collaborate with providers, counselors, and care teams to ensure coordinated and comprehensive care plans.
- Develop and manage individualized care plans, supporting patient needs with internal and external resources.
- Facilitate access to medications by handling prior authorizations and coordinating with insurance providers.
- Respond to patient inquiries, triaging phone calls, managing crises, and coordinating follow-up care.
- Accurately document care actions in the EHR, ensuring compliance with safety and quality standards.
- Participate in quality improvement initiatives to enhance care processes and outcomes.
- Connect patients with community resources, support groups, and additional care services to address holistic needs.
- Full- Time, 0.8 FTE
Qualifications
- Graduate of an accredited Registered Nursing Program.
- Current MN RN licensure in good standing.
- BLS Healthcare Provider certification (must maintain).
- Strong critical thinking and problem-solving skills.
- Ability to work collaboratively and independently in a fast-paced setting.
- Experience in clinic workflows, including EHR management and care coordination.
- Familiarity with medication prior authorizations and community resources.
- Compassionate: Driven to make a meaningful impact on patients' mental health.
- Organized: Skilled at managing multiple priorities and adapting to dynamic needs.
- Collaborative: A team player who fosters positive relationships with colleagues and patients.
- Detail-Oriented: Committed to accurate documentation and process improvement.
- Resourceful: Able to connect patients with appropriate support systems for holistic care.
