Interim HealthCare seeks a responsible Registered Nurse (RN) - Case Manager to oversee our residents’ needs and coordinate with physicians, nurses and other health care providers. We’re looking for a compassionate, care-driven professional who believes in facilitating wellness for patients and their families. The ideal candidate will take initiative in instituting case management policies and communicating with residents in effective ways. The Registered Nurse (RN) - Case Manager (Home Health) responsibilities include:
  • Answer residents’ questions about their care, treatment plans, and all other issues so they feel safe and secure in our care
  • Devise plan of care upon patient admission so that each resident receives consistent, attentive care through the duration of their stay
  • Monitor and adjust patient plan of care based on changing needs and conditions
  • Address any patient concerns with appropriate in-house department
  • Shared on call duties

Job Requirements

Registered Nurse (RN) - Case Manager requirements include:
  • Registered Nurse (RN) license issued by the State of MN
  • Home Care experience preferred
  • Strong written and verbal communication skills
