Buffalo Hospital, located on the western edge of the Twin Cities metro area in Buffalo, Minn., offers high quality, personal care in private rooms and provides many specialty services, including the Birth Center, Cardiac Center, Emergency Services, Sleep Center, Phillips Eye Institute, Courage Kenny Rehabilitation InstituteTM and Virginia Piper Cancer Institute.

The Lead for Care Management:
Functions as a member of the Patient Care Team, providing social work services including assessment, planning, and intervention.
Coordinates the efforts of staff to ensure that functions are performed in an efficient and clinically appropriate manner.
Provides patients with education, advocacy, and decision making support related to the impact of medical illness, mental illness, social needs, functional needs, or financial needs on healing, recovery, chronic care, or end of life care.
Assures appropriate reporting of vulnerability or suspected abuse as mandated by law.
Functions as a primary liaison between staff/patients/families/physicians/clinic, community resources, and long term care facilities.
Coordinates efforts and leads activities related to care coordination of outpatients and UTPs.
Collaborates with the department manager to:Coordinate scheduling and daily staffing
Assist with performance reviews
Coach performance of care management employees
Plan, direct and update policies/procedures/workflows/guidelines/ documentation templates

Assist in coordinating social work internship experiences
Assist with planning staff meetings, as well as involvement in various hospital and system committees and workgroups
Support design and rollout of performance improvement activities

Education

4-6 years post high school education with MSW or LICSW required.

Qualification/Skills

Prior experience in a health care setting preferred with the following demonstrated skills:
Ability to work independently in ambiguous and complex situations with excellent problem identification, problem solving and work prioritization skills.
Effective communication and human relations skills.
Knowledge and skills related to case management/coordination, community resources and complex internal and external health care delivery systems.
Demonstrated strength in evaluating and managing complex situations including conflict and decision management.
Ability to function effectively as part of an interdisciplinary team.
Knowledge of county, state and federal regulations.

Work Schedule

Dept hours 8-4:30, seven days/week, some flexibility of schedule
