Job Details

The Qualified Program Specialist (QPS) is responsible for supporting people in the attainment of their personal outcomes and desires, responsible to assure that each person served is choosing and achieving their Personal Outcomes in their home and community. The QPS demonstrates knowledge of Personal Outcome Measures/CQL and Title 19 regulations. QPS acts as a liaison between the person, his/her family and/or guardian, and other support networks. They are responsible to coordinate annual Person-Centered Support Plan and other meetings as necessary during the Plan Year. They are responsible to identify required and additional skill assessments for Person-Centered Support Plan development. They are responsible to distribute skill assessments annually and update through the Plan year. They are responsible to assist and support people as they identify and work toward their personal goals and outcomes. They are responsible to solicit and respond to the person's served feedback as to their wants and needs as they change. They are responsible to provide in-service training after each Person-Centered Support Plan and its changes during the Plan year. Behavior Intervention Plans will be in-serviced following BIRC direction and with each new Support Staff. They are responsible to monitor plans to ensure that the supports are in place and effective. Quarterly reviews will be conducted to measure and/or assess Outcomes/Processes are being met; this will be documented in the case file, distributed to other team members and entered into a computer database from tracking monthly. Respond timely to persons served for desired changes expressed about their Plan and if progress is not evident upon monitoring. They are someone with a positive attitude and are self-confident, caring, responsible, and dependable. Balances team and individual responsibilities while ensuring quality and timeliness of tasks. Responsibly handles all personal monies and management of benefits. Require the ability to be responsive to the demands or directions of multiple people or interests. Other duties assigned include rotating weekend/holiday and storm tram on-call procedures.

Caseload:
This caseload will include two ICF Group Homes, people supported in an ISLA setting, and people supported in their family home.
This position is required to work at least 4 hours per week of direct care.

Requirements:

B.A. or B.S. degree from an accredited college or university in Human Services field.
At least one year of experience working directly with persons with intellectual or developmental disabilities.
Current and valid driver’s license, proof of auto insurance and completion of motor vehicle report within company guidelines.
Requires successful completion of criminal background, child abuse and neglect checks and pre-employment drug test to be eligible for employment.
DD Certification Preferred.
