The Licensed Social Worker is a Part Time position in a long term care facility. The Social Worker would work with the interdisciplinary team to assess and promote social and emotional health and well-being of residents and their families. The Social Worker provides service to adults and older adults with the goal of affirming the resident's Integrity, growth, and control over the qualities of their life.

Job Responsibilities

• Demonstrates knowledge of skills necessary to identify specific resident needs and provide appropriate services.
• Participates in development of interdisciplinary plan of care.
• Actively develops one-on-one relationships with residents and families as needed to be able to meet individual needs.
• Provides education and resources, as well as support to residents, families and staff.

Requirements

• Bachelor's Degree in Social Work from an accredited college or university.
• Licensed Social Worker in Wisconsin
• Previous experience in long term care or healthcare preferred.
