This is a full-time position working40 hours per week. Time willbe split working with both HomeCare & Hospice.The hours are Monday-Friday from 8am-5pm.



Scope:
  • To serve the patient, family, and facilities by providing clinical information of the home care and hospice continuum.
  • Will travel to patient locations such as private residence, assisted living facility, skilled nursing facility, hospital; anywhere one calls home.
  • Serves as an educational resource for agency, providers, patients, family and facilities.
  • Identifies home care and hospice admissions in the community to enroll in in programs in a timely manner.
  • Collaborates and facilitates positive relationship development among referral sources
  • Participates in developing and enhancing tools and educational programs that promote patient services.

Key Responsibilities:
  • Nurse Navigator Duties - Works closely with Home Care and Hospice Directors regarding criteria for services. Collaborates with employees and facility staff to coordinate care. Attends care conferences as requested. Interacts and facilitates positive relationship development and serves as liaison among community partners. Is accessible via phone and email during working hours. Flexibility in work schedule to accommodate needs of patient, family, and facilities. Facilitate communication of patient status and plan of care during transitional experiences. Participates in developing and enhancing tools and educational programs that promote patient services. Manages time effectively to ensure all duties and documentation requirements are completed promptly. Maintains communication regarding compliance, job performance, and significant patient care and facility issues as they arise
  • Customer First Attitude - Makes the customer experience a primary focus of all actions. Understands that the customer extends beyond the patient and family, including community partners and referral sources, etc. Consistently develops and sustains mutually beneficial working relationships. Is accessible and provides prompt resolution to concerns or problems. Encourages and facilities cooperation within the organization and with customer groups.
  • Interpersonal Communication/Problem Solving - Adopts communication styles appropriate to the listeners and situation. Listens actively, asks questions, clarifies points and rephrases others’ statements to check mutual understanding. Modifies communication in response to feedback from listeners. Works effectively with others to resolve conflict in a constructive way that builds relationships and drives results.
  • Accountability/Leadership - Practices accountability to daily work tasks, expectations and team norms. Identifies what needs to be done and proactively takes action to accomplish objectives and achieve company goals and priorities. Creates a positive culture and working environment within the organization. Inspires, motivates, encourages and guides others toward goal accomplishment. Demonstrates and fosters a sense of team spirit, pride, trust, and commitment.
  • Other Duties as Assigned - The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.

Education and Experience Required:
  • Must complete and pass a state required background study.
  • Must have a car with required insurance coverage and a state driver’s license to travel to clients’ residences.
  • Must have adequate computer skills.
  • Current Registered Nurse licensure in the state of North Dakota and Minnesota.
  • Current BLS certification.
  • Able to communicate verbally and in writing to the extent required by the position.
  • Able to physically perform the duties required by the position.
  • Required to have the ability to read, write and speak English well enough to communicate with staff, patients, families and facilities.
