JOB SUMMARY

Answers patient inquiries over the telephone, assessing the severity of the patient's symptoms and medical needs according to triage algorithms. Guides the patient to the appropriate level of care; schedules patient appointments over the phone with appropriate healthcare providers/facilities/physicians. Documents information provided to patients during triage in health record systems; records patients health conditions for follow-up patient care. Maintains and develops triage protocols; ensures patients' healthcare information is updated on time. Role does answer inquiries through multiple variations of technology, depending on department. Educate patients based on calls that come in.

POSITION RESPONSIBILITIES

Explains treatments and procedures to patients suffering less serious illnesses and injuries. Delivers written and oral communication responds to questions and concerns, and produce specific outcomes and impact. Coordination and management of care for patients, including with complex needs. Builds a rapport with the patient and explains the condition and course of action effectively. Responsible for promoting a safe environment for patients, families, staff, residents, and visitors and working collaboratively with the multi-disciplinary health care team to provide the best outcomes for patients. Provides direct service to internal or external customers. Performs triage, sorting patients needing urgent care from those with less severe injuries. Performing age related patient assessments and nursing interventions. Effectively delegates and supervises based on one's state practice act and an understanding of the concepts of responsibility, authority, and accountability.

DEPARTMENT DETAILS

The Family Medicine Resident Clinic is open Monday - Friday 8 - 5. This clinic hosts 16 residents who work with us for 3 years while completing their physician training. In this role, you will work closely with the physicians and residents to provide care and treatment to our patients. This clinic typically has a lower patient volume, which allows for more training time with the providers and residents. Here, you get to assist not only with patient education but resident education!

QUALIFICATIONS

Graduate from an accredited nursing program preferred, including, but not limited to, American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelor's Degree in nursing preferred.
One year of healthcare experience preferred.
Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Nurses performing nursing practice over the telephone require licensure in the states where the patients being served reside. Within first 30 days of employment, all requirements to obtain license needed for role is completed. Obtains and subsequently maintains required department specific competencies and certifications.
