JOB SUMMARY

The Clinical Care Leader (CCL) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care in the hospice setting. Collaborates with other inter-professional colleagues, to plan, implement and evaluate care. Documents patient history, symptoms, medications and care provided in compliance with applicable standards/regulations. Utilizes technology to enhance quality patient care, collaborates with inter-professional team members, effectively delegates and supervises unlicensed personnel, assumes overall nursing accountability for a patient assignment and serves in leadership roles on department/organizational tasks forces and committees. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in the specific care area. Communication skills to collaborate, negotiate and resolve conflicts, situations, and/or issues with peers, inter-professional colleagues, patients and families. Must have strong clinical reasoning and critical thinking skills to analyze data and appropriately respond to changing situations, coordination of care, patient assessment, patient education, triage, and various other nursing interventions. Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in the state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.

POSITION RESPONSIBILITIES

Effectively delegates and supervises based on one’s state practice act and an understanding of the concepts of responsibility, authority, and accountability.;Educates and trains others on operations, ethics, and regulations within the home healthcare industry.;Applies home healthcare operating principles and practices within own position.;Applies general clinical skills to perform clinical functions or tasks.;Adjusts departmental priorities and resource allocations to support changing needs.;Uses active listening skills and probing techniques to understand cause of a conflict.;Seeks opportunities to serve as a mentor.;Responsible for promoting a safe environment for patients, families, staff, residents, and visitors and working collaboratively with the multi-disciplinary health care team to provide the best outcomes for patients.;Reinforces positive contributions and recommends adjustments for improving outcomes.;Promotes an organizational culture that balances individual initiative and team collaboration and sharing of successes.;Fosters strong customer relationships via delivery on commitments, open communication, and on-going feedback/improvement.;Establishes and maintains productive working relationships within and outside of own area.;

DEPARTMENT DETAILS
70% is patient care and 30% is office duties. Resolving scheduled visit issues, monitoring for open visits or uncompleted visits. Chart audits. Educating new team members. Train to be a super trainer for hospice charting. This role requires someone that is not afraid to help coach team members that deviate from processes or policy. Someone that is eager to uplift the team and promote good communication.
QUALIFICATIONS

Bachelor's degree in nursing strongly preferred. Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
Minimum of two years home health/hospice nursing experience preferred.
Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications. Valid driver's license and proof of insurance required. Basic Life Support (BLS) required. Certificate for Certified Hospice and Palliative Nurse (CHPN) preferred.
