Job Summary

The Registered Nurse (RN) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care to residents. Collaborates with resident and family, other inter-disciplinary colleagues, including providers, to plan, implement and evaluate care. The nursing process is utilized to assess, diagnose, establish a plan with outcome criteria, intervene, evaluate, and document human responses to actual or potential health problems. This practice includes dependent, interdependent and independent functions.

Neilson Place is a 78 bed Skilled Nursing Facility located in beautiful Bemidji, MN. Built in 2004, Neilson Place was founded on the social model, culture change and creating a home atmosphere for area seniors to live and receive care. Neilson Place consists of three long term care neighborhoods and one post-acute neighborhood where people come to receive short term clinical and rehabilitation services. Neilson Place is part of the Woodsedge Campus which consists of Windsong - an 80 apartment senior living community with assisted living services; and Trillium - a 27 apartment memory care assisted living. In addition to a full senior services continuum of care, Neilson Place is an integral part of the Sanford Bemidji Health System and works hand in hand with the hospital, clinic, home care/hospice, behavioral health and specialty providers.

Qualifications

Must have graduated from an accredited school or college of nursing. Bachelor's degree in nursing is preferred.
Specific services/positions may have additional education, training or experience requirements. Must have attended a pre-licensure nursing program that included supervised clinical experience across the life span as part of the curriculum or have had a minimum of 400 hours of experience as a RN (or LPN) at another facility.
Requires current licensure as a Registered Nurse in the state, or states, of practice. Nurses performing nursing practice over the telephone require licensure in the states where the patients being served reside. Certification may be required depending on specialty or service area.
