JOB SUMMARY

RN - The Registered Nurse (RN) isresponsible for utilizing the nursing process (assessment, diagnosis,outcomes/planning, implementation and evaluation) to provide individualizednursing care to residents. Collaborates with resident and family, otherinter-disciplinary colleagues, including providers, to plan, implement andevaluate care.

Demonstrates competency andpractices within the full scope of nursing expertise/knowledge and utilizesappropriate age and population specific standards as designated in theirassigned clinical setting. Cares for residents in all phases of preventativecare, health maintenance, diagnosis, and treatment. Responsible for thecoordination of care, resident assessment, resident education and various othernursing interventions.

May serve as charge nurse and delegate appropriate tasks to license andunlicensed assistive personnel that are within the job descriptions, skills,resident care needs, and the competence level of the delegate. Providesadequate assistance and support to delegates to ensure safe, reliableadministration of resident care and resolution of care and service concerns.Functions within the scopes and standards of nursing practice as outlined inthe Nurse Practice Act and Administrative Rules in state of practice andlicensure. The Sanford Professional Nursing Practice recognizes the Scope andStandards of Practice and the Code of Ethics for Nurses with InterpretiveStatements as published by the American Nurses Association as the foundation ofnursing care delivery and professional conduct.

LPN- The Licensed Practical Nurse (LPN) provides professional nursing care forresidents of all ages in long term care, under the supervision of a registerednurse, advanced practice provider, or physician.

Demonstratesknowledge of nursing principles for the resident in the relevant clinicalsetting. Demonstrates competency and practices within the full scope of nursingexpertise/knowledge and utilizes appropriate age and population specificstandards related to the physical and psychosocial needs of the resident as percare plan. Possess the ability to perform testing to evaluate data reflectiveof the resident's status and to provide the care needed. Demonstrates abilityto recognize problems, recommend solutions and respond effectively to assist inan emergent situation. Ensures the health, comfort and safety of residents.Contributes to the assessment of residents and administers medications ortreatments as ordered. Provides technical support to healthcare professionalsas needed. Documents resident interactions as needed and the outcomes relatedto resident care, medication, behavior, activity, and dietary. Skilled inestablishing and maintaining effective working relationships with resident,health care providers, and the public.Ability to work effectively with professional and supportive personnel as amember of an interdisciplinary care team. Ability to communicate clearly andmaintain confidentiality of sensitive information. Counsels residents, familymembers and/or resident advocate, under the direction of a registered nurse,advanced practice provider or physician on prevention and treatment plans.Knowledge of resident education principles to develop a self-care program.Performs other related duties as assigned.

POSITIONRESPONSIBILITIES

RN - Performs agerelated patient assessments and nursing interventions. Monitors patient’sresponse to treatment plan and adjusts accordingly. Monitors patient’s healthconditions and records all medical related data, e.g. progress notes.Effectively delegates and supervises based on one’s state practice act and anunderstanding of the concepts of responsibility, authority, and accountability.Documents current patient medication, medication administered, andreactions/results. Delivers written and oral communications that engageaudience participants, respond to their questions and concerns, and producespecific outcomes and impact. Coordination and management of care for patients,including with complex needs. Adjusts clinical practices according to patients’wishes, keeping within policy limits. Adheres to legal regulations andorganizational policies when discussing patient health information. Responsiblefor promoting a safe environment for patients, families, staff and visitors andworks collaboratively with the multi-disciplinary health care team to providethe best outcomes for patients.

LPN - Adheresto legal regulations and organizational policies when discussing patient healthinformation Applies home healthcare operating principles and practices withinown position Communicate relevant information with patients and health teammembers to ensure quality and continuity of care. Contribute to the evaluationof the plan of care by gathering, observing, recording and communicationpatient responses to nursing interventions. Effectively delegates andsupervises based on one’s state practice act and an understanding of theconcepts of responsibility, authority, and accountability. Establishes andmaintains productive working relationships within and outside of own area.Follows safety measures to

ensure proper use of equipment andsupplies. Implement interventions that are assigned, ordered or prescribed bylicensed practitioner or registered nurse. Observe patients, document andreport changes in patient condition, such as adverse reactions to medication ortreatment, and take any necessary action. Participate and be part of ahealthcare team to conduct focused nursing assessments and contribute to the planof care. Participate in health teaching developed by a licensed practitioner orregistered nurse. Promote a safe and therapeutic environment by providingappropriate monitoring and surveillance of the care environment. Responsiblefor promoting a safe environment for patients, families, staff, residents, andvisitors and working collaboratively with the multi-disciplinary health careteam to provide the best outcomes for patients.

QUALIFICATIONS

RN - Graduate from a nationallyaccredited nursing program preferred, including, but not limited to, Commissionon Collegiate Nursing Education (CCNE), Accreditation Commission for Educationin Nursing (ACEN), and National League for Nursing Commission for NursingEducation Accreditation (NLN CNEA). Bachelor's Degree in nursing preferred.

Currently holds an unencumbered RN license with the State Board of Nursingwhere the practice of nursing is occurring and/or possess multistate licensureif in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintainsrequired department specific competencies and certifications.

LPN - Graduate from anaccredited practical nursing program as a Licensed Practical Nurse. Associatedegree in practical nursing is preferred.

Must have attended a pre-licensurenursing program that included supervised clinical experience across the lifespan as part of the curriculum or have had a minimum of 400 hours of experienceas LPN at another facility. Specific services/positions may have additionaleducation, training or experience requirements.

If position requires IVtherapy/management skills, the following is required in North Dakota:graduation from a Board approved AASPN program where IV therapy is part of thecurriculum or completion of an IV Therapy course approved by the North DakotaBoard of Nursing.

Currently holds an unencumbered LicensedPractical Nurse (LPN) license with State Nursing board and/or possessmulti-state licensure privileges. Obtains and subsequently maintains requireddepartment specific competencies and certifications. Completes required annualcontinuing education units (CEU) and competency training requirements of thedepartment and facility.
