Job Responsibilities

Manages the Continuous Survey Readiness Process (CSRP) by assigning and following up on progress for completion of audits and data reviews; facilitates the creation and completion of action plans for deficient areas. Assigns quality rounds and focus audits tasks and follows up on progress for completion. Reviews the data and facilitates the creation and execution of action plans for deficient areas. Facilitates the QAPI Committee, ensures that QAPI addresses all systems of care and management practices, including clinical care, quality of life, resident autonomy and choice and safety. Appropriately initiates Performance Improvement Projects (PIPs), following the Society's Model for Improvement process. Ensures the annual QAPI assessment is completed and coordinates the formulation of the annual QAPI plan. Reviews and analyzes data from a variety of sources, including but not limited to CSRP and other audits, Dashboard, PCC and CareWatch; creates action plans to address identified issues. Provides training on PIP, Continuous Survey Readiness and other QAPI endeavors. Communicates progress of QAPI work via meeting minutes.

Qualifications

College degree preferred, prior long-term care experience preferred; prior CQI/QA experience highly preferred

Ability to measure and comprehend and interpret numbers, statistics and graphs; ability to learn and use computers and software programs to create reports and maintain facility QAPI records; ability to plan, direct and conduct in-service or just-in-time training related to QAPI

Familiarity with state/federal long-term care regulations preferred
