This is a Monday-Friday 8am-4:30pm with one day off a week. No weekends or holidays. P rimarily serving the Eastern side of our Park Rapids territory (Walker, Hackensack, Akeley areas).

Scope:

• Responsible for delivery of physical therapy services in accordance with the standards & practices of physical therapy, state & federal regulations and Knute Nelson’s policies and procedures with minimal supervision.
• Assure that the highest degree of quality care, customer service, fiscal responsibility, and regulatory compliance is maintained at all times.
• Receive clinical direction and supervision from licensed Physical Therapists.

• Direct Client Care – Implement a planned treatment program designed by the Physical Therapist by utilizing exercise, physical agents, assistive and/or supportive devices & other treatment procedures & equipment. Directly assist the Physical Therapist in performing complex evaluation and/or treatment procedures.
• Client Records and Billing – Provide accurate and timely documentation of services rendered. Document in EMR at the client visit in home. Demonstrate understanding and knowledge of all payer sources. Complete accurate and timely submission of billing information & appropriately account for all services rendered. Responsibility to chart per agency policy and Medicare regulations.
• Communication – Communicate effectively with client, care givers, and family in order to establish meaningful, functional goals. Communicate effectively with the interdisciplinary team regarding client progress, problems, and functional outcomes within appropriate confidentiality guidelines. Demonstrate exceptional verbal and written communication skills.
• Other Duties as Assigned – The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.

Education and Experience Required:

• Must be licensed as a Physical Therapist Assistant in the state of Minnesota.
• Must obtain CPR certification within 6 months from date of hire.
• Must have a valid driver’s license and use of an insured personal vehicle.
• Must complete and pass a state required background study.
• Able to communicate verbally and in writing to the extent required by the position.
• Able to physically perform the duties required by the position.
• Required to have the ability to read, write and speak English well enough to communicate with staff, residents and visitors.
