Job Summary

Join our not-for-profit organization that has provided nearly 100 years of housing and service, now including skilled home health services with a commitment to quality care and service. This position's primary service area is Denton, TX.
Job Responsibilities
Performs skilled home health assessments; OASIS documentation and visit notes; care plan development; communication with physician, care team members, patient and family. Provides direct patient care including infusion therapy, wound care, teaching, and disease management. Supervises LVN and Aide assigned. Coordinates care with other disciplines and providers. Participates in on-call schedule. Provides care in accordance with agency policy; local, state and federal laws and regulations. Follows infection control guidelines. Complies with HIPAA guidelines. Participates in case conference and QAPI processes.
Qualifications
Graduate of an accredited school of nursing. RN license in state of Texas.
Excellent nursing and supervisory skills. Excellent organization and communication both verbal and written.
Familiarity with the therapeutic effects and side effects of medications.
Prior work experience in like or related position preferred.
