Job Responsibilities
This position provides care to assigned residents in a caring, safe and efficient manner and is responsible to perform the following according to location standards, procedures and individualized resident care plans
- Completes the assigned portions of the MDS for Nursing.
- Signs the MDS forms to signify completion of the process.
- Maintains residents' self-esteem, privacy and confidentiality of personal information.
- Assists to maintain safe, pleasant, orderly resident living areas.
- Completes nursing portion of MDS and reviews for completion and validity of entire process.
- Queries other members for accuracy of MDS; and provides final quality check prior to MDS transmission.
- Receives and shares information.
- Observes and reports residents' conditions.
- Communicates with physicians using center/campus systems.
- Directs and assists with co-worker, resident, family/significant other education.
- Provides support and recognition to co-workers related to completion of the RAI / MDS process. Uses the EMR accurately.
Qualifications
- Graduate of an accredited school of nursing.
- Must be certified through AANAC as a Resident Assessment Coordinator (RAC-CT) within 6 month of hire.
- Ability to communicate and comprehend.
- Knowledge of the RAI/ MDS process; knowledge of basic nursing skills.
