Job Responsibilities

This position provides care to assigned residents in a caring, safe and efficient manner and is responsible to perform the following according to location standards, procedures and individualized resident care plans
  • Completes the assigned portions of the MDS for Nursing.
  • Signs the MDS forms to signify completion of the process.
  • Maintains residents' self-esteem, privacy and confidentiality of personal information.
  • Assists to maintain safe, pleasant, orderly resident living areas.
  • Completes nursing portion of MDS and reviews for completion and validity of entire process.
  • Queries other members for accuracy of MDS; and provides final quality check prior to MDS transmission.
  • Receives and shares information.
  • Observes and reports residents' conditions.
  • Communicates with physicians using center/campus systems.
  • Directs and assists with co-worker, resident, family/significant other education.
  • Provides support and recognition to co-workers related to completion of the RAI / MDS process. Uses the EMR accurately.

Qualifications
  • Graduate of an accredited school of nursing.
  • Must be certified through AANAC as a Resident Assessment Coordinator (RAC-CT) within 6 month of hire.
  • Ability to communicate and comprehend.
  • Knowledge of the RAI/ MDS process; knowledge of basic nursing skills.
