Job Responsibilities
- This position provides care to assigned clients in a caring, safe and efficient manner and is responsible to perform the following according to location standards, procedures and individualized client plans of care.
- Defines the unit of care (family and client). Demonstrates knowledge of hospice palliative care; hospice philosophy and scope of services; importance of coordinating services and continuity of care.
- Recognizes different cultural attitudes and values towards death and dying. Identifies the five stages of the grieving process; anticipatory grief; normal vs. abnormal grief and impact of grief on client's family. Provides care according to plan of care and evaluates clients' need for additional services.
- Involves client and family in creation of plan of care. Coordinates with interdisciplinary team as needed. Completes required documentation in appropriate timeframes. Explains advance directives; clinical requirements and interdisciplinary services available through agency.
- Works independently with minimal supervision and seeks guidance as needed. Performs initial client assessment and ongoing assessments of the client's status. Participates in creation of the plan of care.
- Ensures the plan of care is implemented by the interdisciplinary group. Processes physician services/orders. Completes quality assurance audits as assigned.
- Participates in agency in-services; educational opportunities; team meetings and committees. Assists with customer experience/marketing activities to expand community reach.
Qualifications
- Graduate of an accredited school of nursing. Licensure for state of practice is required.
- Must be at least 18 yrs of age.
- Prior work experience in like or related position may be helpful.
