Job Responsibilities

This position provides care to assigned residents in a caring, safe and efficient manner and is responsible to perform the following according to location standards, procedures and individualized resident care plans.
  • Defines the unit of care (family and client).
  • Demonstrates knowledge of hospice palliative care; hospice philosophy and scope of services; importance of coordinating services and continuity of care.
  • Recognizes different cultural attitudes and values towards death and dying.
  • Identifies the five stages of the grieving process; anticipatory grief; normal vs. abnormal grief and impact of grief on client's family.
  • Provides care according to plan of care and evaluates clients' need for additional services.
  • Involves client and family in creation of plan of care.
  • Coordinates with case manager/supervisor, as needed, prior to a client visit.
  • Coordinates with physician and interdisciplinary team as needed.
  • Completes required documentation in appropriate time frames.
  • Explains hospice social work requirements and social work services available through agency.
  • Works independently with minimal supervision and seeks guidance as needed.
  • Verbalizes understanding of appropriate and coordinated Social Work services to families/caregivers.
  • Participates in the development of the plan of care.
  • Assesses client status and completes social evaluation of client and family needs.
  • Assesses for, and educates interdisciplinary group, on any special needs related to the culture of the patient and family. Includes communication, role of family, space, and any special traditions or taboos.
  • Provides referral information to client and client family/caregivers based on the needs in the evaluation results and acts as a liaison between the client and community agencies.
  • Provides counseling and crisis intervention as needed to client and family/caregiver.
  • Completes quality improvement audits as assigned.
  • Assists client and family with planning of funeral services, coordinating financial, legal and health care decisions.
  • Assists clients and family/caregiver with discharge planning and/or level of care change as needed.
  • Participates in agency in-services; educational opportunities; team meetings and committees.
  • Assists with customer experience/marketing activities to expand community reach.

Qualifications
  • Qualifications include a graduate of a bachelor's program in social work accredited by the Council on Social Work Education or meets personnel qualifications stated in 418.114(b)(3).
  • Licensure or certification as a social worker in the state where the program is located, if required by State Social Work licensing board.
  • Must be licensed with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Minimum of one (1) years' experience in health care, hospice experience preferred.
  • Must have an Arizona Fingerprint Clearance Card
  • Must have a CPR Certification
