Job Responsibilities
  • This position provides care to assigned clients in a caring, safe and efficient manner and is responsible to perform the following according to location standards, procedures and individualized care plans.
  • Collaborates with location leadership on implementation of clinical programs and projects. Assists hospice location employees in maximizing client quality of life.
  • Participates in the Quality Assurance Performance Improvement (QAPI) program and performance improvement project teams. Evaluates clients who have acute changes in condition, and collaborates with the primary care provider and employees to implement appropriate interventions.
  • Documents medical care provided. Assures primary physician is notified of clients' condition; and requests and orders are properly processed.
  • Actively participate in assuring safe transitions of care, on admission, transfer, or discharge. Work with interdisciplinary teams to assure the needs of hospice clients, client family members and caregivers are supported.

Qualifications
  • CPR certification is required; graduate of an accredited RN program and licensed to practice as an RN and NP for the state of practice is required,
  • Master's Degree required; Board certification as a nurse practitioner is required. DEA licensure or ability to obtain.
  • Must have the ability to lead and motivate individuals or groups of people toward the accomplishment or work and organizational goals. Must have the ability to communicate verbally with employees, physicians and other outside professionals effectively.
  • Skill and compassion in communicating effectively with clients and client family members. Ability to recognize different cultural attitudes and values toward death and dying.
  • Ability to identify the five stages of the grieving process; anticipatory grief, normal vs. abnormal grief, and impact of grief on the family.
  • Minimum two years' experience as a nurse practitioner preferred. Hospice setting experience preferred. Demonstrates knowledge of hospice palliative care, philosophy and scope of services.
