Job Responsibilities

This position provides care to assigned residents in a caring, safe and efficient manner and is responsible to perform the following according to location standards, procedures and individualized resident care plans.

Participates in pre-admission visits and admission process for unit residents. Participate in development of interdisciplinary discharge plans for residents. Assures successful transfer of residents both within the center/campus and to outside agencies. Assures leave of absences are processed smoothly. Completes assessments and formulates interventions and programming to address individual needs. Assures plan of care, programming and interventions are effective. Provides individual or group support, referral and advocacy services for residents and/or families. Assures implementation of daily programming; leads or assists with conducting programming. Assists residents in maintaining proper and clean appearance. Receives and shares information; observes and reports residents' conditions to appropriate staff. Documents residents' programming, activity participation, completes all documentation within established timelines; provides input to evaluation and care planning for residents; measures body functions and documents care provided to residents. Assures community involvement with the unit and works with volunteers; serves as primary liaison between the unit and community; markets the unit's services.

Qualifications

Bachelor degree in therapeutic recreation, social work, nursing, gerontology, occupational therapy, psychology or another human services field is preferred. Prior work experience in long term care with individuals with Alzheimer's Disease/dementia is preferred; prior supervisory experience. Licensure or certification by a recognized accrediting body in the state serviced.

Licensure for state of practice is required.

Demonstrated knowledge of caring for people with special needs; ability and desire to be creative and ability to adapt programs and services to meet individual resident needs.
