Job Summary
  • Shifts are Monday through Friday including 1 weekend every two months and 1 holiday per year

Job Responsibilities
  • This position assists the social worker with resident admissions, transfers and discharges; completes documents as assigned
  • Contacts appropriate community members, speaks at community functions and conducts tours and educational sessions; keep community aware and up to date on the care center/campus
  • Assists with interdisciplinary discharge plans for residents
  • Assures successful transfer of residents both within the center/campus and to outside agencies; assures leave of absences are processed smoothly
  • Assures admissions process of residents is done in compliance with state and federal regulations along with GSS guidelines
  • Assists Social Worker in providing or arranging for social services for residents/ family members as appropriate
  • Completes documents related to social work as designated by the Social Worker or Administrator
  • Assists with training staff on issues related to resident care, rights and behaviors
  • Uses the EMR accurately

Good Samaritan Society-Canistota has a Behavior Program contracted with South Dakota's Department of Social Services. The Behavior Program allows the center to admit elderly residents with mental illnesses/advanced dementia with behaviors. The program has a 15-resident capacity. This position leads this program.

Qualifications
  • Must have the ability to communicate with the elderly and persons with disabilities
  • Must have the ability to become a Licensed Social Work Associate in the state of South Dakota. Which includes

-Baccalaureate degree in a nonsocial work field or discipline or an associate of arts degree in the human services in a program approved by the Board from a junior college, college, or university approved by the Board

-Pass an examination prepared by the Board for this purpose
