Job Responsibilities

This position provides care to assigned clients in a caring, safe and efficient manner and is responsible to perform the following according to location standards, procedures and individualized client care plans.
  • Defines the unit of care (family and client).
  • Demonstrates knowledge of hospice palliative care; hospice philosophy and scope of services; importance of coordinating services and continuity of care.
  • Recognizes different cultural attitudes and values towards death and dying. Identifies the five stages of the grieving process; anticipatory grief; normal vs. abnormal grief and impact of grief on client's family.
  • Provides care according to plan of care.
  • Coordinates with the Case Manager/Supervisor prior to visit as appropriate.
  • Coordinates with interdisciplinary team as needed.
  • Completes required documentation in appropriate timeframes.
  • Explains the certified aide role in the client's plan of care; purpose for the visit; the certified aide services offered by the agency; explains client rights and involves client family members in care.
  • Displays knowledge regarding hospice policies and procedures related to certified aide services.
  • Works under the direction of the Registered Nurse and seeks guidance as needed.
  • Performs personal cares according to clients' plan of care.
  • Responds quickly to client requests.
  • Uses positive communication with client and family and respect for client, client family/caregiver and personal needs and environment.
  • Uses equipment properly and safely.
  • Assists client with dressing/undressing, performing personal cares (baths, back rubs, oral hygiene, grooming, changing bed linens and other light laundry).
  • Plans and prepares meals and assists client with feeding according to the client plan of care.
  • Accurately takes and records client's temperature, pulse respiration and blood pressure according to client plan of care.
  • Assists with ambulation and exercise, range of motion and repositioning as instructed by the agency nurse.
  • Provides care to avoid skin breakdown.
  • Assists clients with toileting, bedpan and urinal use as assigned.
  • Reports any changes to client's conditions to the case manager.
  • As directed by state law and the client's plan of care, reminds clients to take medication.
  • Keeps client's living area neat.
  • Provides respite care for client's care givers as appropriate.
  • Participates in agency in-services; educational opportunities; team meetings and committees.
  • Assists with customer experience/marketing activities to expand community reach.

Qualifications
  • High school education or higher education is not required to be hired into this job.
  • Completion of an accredited nursing assistant / HHA training program with written skills test and competency evaluation, and formal certificate obtained.
  • Must be at least 18 years of age.
  • At least six months prior work experience in a hospice.
  • Arizona Fingerprint Clearance Card
  • CPR Certification required
  • Reliable transportation plus Valid driver's license and proof of current auto insurance.
