The Lead Panel Assistant is the primary point of contact for panel assistant staffing. Greatly contributes to the success of the medical home model of care that originates within care clinics by ensuring overall quality patient care. Ensures overall treatment and patient plans are understandable for all involved, processes are streamlined for patients, and care is fully supported by the entire care team. Provides expert knowledge and assistance in the clinic care team in managing and coordinating patient care. Provides guidance and direction regarding workflows. Oversees the coordination of provider's patient panels by monitoring patient attribution, wellness screenings for specific specialties, schedule office visits, and managing the appropriate chronic disease registries.
Participates in designing and promoting quality initiatives and improved patient outcomes. Roll out new and updated policies, procedures, and workflows to team while championing overall changes. Participates in the care of patients by collecting subjective and objective data from the patient or caregiver. Communicates collected data to the health care team and follows through on the patient's plan of care under the direction of the care team. Orientates and trains new staff with advanced knowledge in setting up registries and appropriate maintenance.
Demonstrates computer literacy including using Microsoft Office products and electronic health record (EHR). High level of computer skills and ability to run registries. Participates in care for age related patients in all phases of preventative care and health maintenance.
Functions within the administration pre-defined scope of practice guidelines. Participates and represents additional assigned workgroups and committees for overall process improvement and quality assurance, while assisting in running staff meetings as well. Demonstrates experience and professionalism in the following skills, but not limited to: customer service, critical thinking, multi-tasking, medical and medication terminology, and medical paperwork knowledge including insurance completion/submission requirements.
Post secondary education with an emphasis in healthcare or medical related field is preferred.
Minimum one year experience as a panel assistant.
Background as a Certified Medical Assistant (CMA) is preferred.
