Public Health Nurse

Horizon Public Health is seeking applicants for a Full Time Registered Nurse to work as a Certified Assessor/Case Manager in our Alexandria office.  A Certified Assessor is responsible for conducting assessment interviews, determining level of care eligibility for waiver programs, and developing community support plans.  A Case Manager provides care coordination, including developing, implementing and evaluating client plan of care, coordinating care using available resources and maintaining contact with care givers.

Minimum qualifications include a Baccalaureate degree in Nursing and licensed in MN.  Exceptions may apply for AD nurses with experience in the areas listed below.  Preferred qualifications 1-3 years’ experience in public health, case management, working in homecare or with long term care services. Successful applicant must pass a criminal background check and a pre-employment drug test.  Must have valid MN driver’s license and reliable, insured transportation. This is a Monday-Friday daytime hours, Grade 13 MNA/AFSCME Council #65 union position, Step 1 $29.24/hour (2021 scale).
