Scope
- Provide
personal care and related services to clients in their home as listed by home
health aide’s plan of care. - Has
frequent contact with clients and family/caregivers, and various members of the
community, and other staff. - Tasks
to be performed must be assigned by and performed under the supervision of a Registered
Nurse (RN) who is responsible for client care provided by the Home Health Aide.

Education and Experience Required
- Must
complete and pass a state required background study. - Must
be registered on the Minnesota and North Dakota Nursing Assistant Registry. - Current
BLS certification. - Must
have a car with required insurance coverage and a state driver's license or
reliable transportation. - Must
pass a skills test prior to performing client care. - Undergo
successful orientation and participate in competency-based assessments of
skills on a regular basis. - Possess
a professional attitude to establish and maintain a good work relationship with
the client, the client's family, and the personnel of the Agency. - Have
a sincere interest in caring for the sick and disabled at home. - Abide
by the HIPAA (Health Insurance Portability and Accountability Act) privacy
policy as administered by Ethos. - Able to physically perform the duties
required by the position. - Required to have the ability to read,
write and speak English well enough to communicate with staff, clients and
visitors.
Key Responsibilities
- Physical Well
Being - All care is provided under the direction of the RN. The Home
Health Aide will follow the care plan for every client. Assist clients to
maintain good personal hygiene. Assist in maintaining a healthful, safe
environment. Plan and prepare nutritious meals. Assist the client with
ambulation. Assist with certain treatments as ordered by the physician
(treatments will also be approved and supervised by a registered nurse). Assist
the therapy personnel as needed with rehabilitative processes. Encourage the
client to become as independent as possible according to the nursing care plan. - Supports the
Client’s Mental Well Being - Attempt to promote client's mental
alertness through involvement in activities of interest. Give simple emotional
and psychological support to the client and other members of the household.
Establish a relationship with client and family which transmit trust and
confidentiality. - Documentation and
Assessment - Prepare documentation per Agency policy. Report any change in the
client's mental or physical condition or in his home situation to supervising
staff nurse and Director of Home Care. Carry out assignment as instructed by
the nurse and reports to the nurse. Confirm on a weekly basis, the scheduling
of visits with the scheduling coordinator and with other personnel.
- Continuous
Improvement - Attend in-service as required by state and federal regulations.
Confirm on a weekly basis, the scheduling of visits with the Supervisor, to
coordinate necessary visits with other personnel. Attend IDT meetings as
scheduled. - Leadership
and Organizational Skills - Show
exemplary qualities in attitude, effort and work performance. Work within
parameters of budget and resources available. Evaluate outcomes of therapy
interventions. Complete additional duties, projects, in-services, promotional
events and program development as assigned by Director of Home Care. - Safety
- Follow safety procedures for work environment
and promote client safety while performing direct care. - Other Duties as
Assigned - Work with personnel of other community agencies involved in the
client's care as directed by the staff nurse. Perform routine homemaking tasks
as related to a safe and comfortable environment for the client, as instructed
by the professional nurse. Minimal shopping when requested to do so by a nurse.
