This is a Full Time 32 hours a week, 64 hours a pay period position.

Scope

·
Frequent contact with clients and
family, and various members of the community, and other staff.

·
Under supervision of an RN, will perform
duties essentially of personal care and palliative care, which would be given
to terminally-ill persons in their homes.

·
Help the client’s family members to
carry their share of the responsibilities and encourage the terminally ill
person to participate in the life of the family, as able.

Education
and Experience Required

·
Must complete and pass a state required
background study.

·
Must be registered on the Minnesota and
North Dakota Nursing Assistant Registry.

·
Current BCLS certification.

·
Must have a car with required insurance
coverage and a state driver's license or reliable transportation.

·
Have the maturity necessary for
establishing and maintaining a good work relationship with the client, the
client's family, and the personnel of the Agency.

·
Have a sincere interest in caring for
the sick and disabled at home.

·
Undergo successful orientation and
participate in competency-based assessments of skills on a regular basis.

·
Must be compassionate and believe in
providing comfort-oriented rather than curative care.

·
Able
to physically perform the duties required by the position.

  • Required to have the
    ability to read, write and speak English well enough to communicate with
    staff, clients and visitors.


Key
Responsibilities

·
Support the client's physical
well-being. All care provided is done under the direction of the RN, the HH
Aide will follow the care plan for every client. Help the client to maintain
good personal hygiene. Assist in maintaining a healthful, safe environment.
Plan and prepare nutritious meals. Assist the client with ambulation. Assist
with certain treatments as ordered by the physician (treatments will also be
approved and supervised by a registered nurse). Assist the therapy personnel
as needed. Respond to the physical, mental, social and emotional needs of
terminally ill patients and their families within scope of practice. Take
accurate vital signs.

Attempt to promote client's mental
alertness through involvement in activities of interest. Give simple emotional
and psychological support to the client and other members of the household.
Establish a relationship with client and family which transmit trust and
confidentiality.

·
Prepare documentation per company
policy. Report any change in the client's mental or physical condition or in
his home situation to his/her immediate supervisor, the staff nurse. Carry
out assignments as instructed by the nurse and reports to the nurse. Confirm
on a daily basis, the scheduling of visits with the Supervisor. Coordinate necessary
visits with other personnel.

Work as part of the interdisciplinary
team and communicates with the IDG regarding the patient visits. Notify
appropriate licensed staff when patient complains of pain or other physical
and emotional changes. Address and support cultural practices, as long as
such practices do not harm others. Treat patients and their families with
respect, dignity and compassion. Perform all aspects of patient care in an
environment that optimizes patient safety and reduces the likelihood of
medical/health care errors. Support and maintain a culture of safety and
quality.
