To provide coordination and implementation of program requirements for homeless and at-risk youth while assisting and keeping with the overall goals of the department and Fraser, Ltd.
Essential Job Functions and Responsibilities
PROGRAMMING
• Conduct Bio/Psycho/Social assessments using accepted and available tools.
• Facilitate weekly individual treatment planning.
• Monitor mental and behavioral health of individuals while responding to barriers that arise following assessment.
• Advocate on clients’ behalf in areas of access to services, support, and accommodation; assist clients in acquiring available
resources, working on barriers to employment, independent living skills, and other areas as identified as concerning; explain
procedures, rights, responsibilities.
• Maintain case notes and documentation for client records.
• Refer clients to collaborating partners when presenting issues are outside the scope of practice.
• Participate in individual observation.
• Participate in bi-weekly team meetings.
MANAGEMENT AND DEVELOPMENT
• Track client goals and progress in behavior through use of appropriate case notes.
• Assist in the development of the department within Fraser, Ltd; submit reports, as directed.
• Provide supervision and training to Direct Service Professionals, as needed.
• Assist Administrator in grant writing and completing grant reports
• Form collaborative relationships with community groups for the efficient and effective delivery of services; assist with the
development of programs with partner agencies and groups to eliminate gaps in services and reduce duplication of offered services.
• Work with agency staff, direct volunteers and interns placed in TYS.
• Attend meetings for administrative, supervisory, and program planning.
• Maintain applicable VI-SPDAT data and applicable housing materials for PSH.
• Meet with Administrator and staff, as warranted; provide case management updates.
• Assist with staff interviews.
• Participate in on-call rotation.
Supervisory Responsibilities
This position coordinates services and trainings for PSH.
Additional Duties, Responsibilities and Qualifications
Others as assigned.
Education and/or Experience
• Master’s degree and current licensure preferred in the field of Social Work or professional supervision that allows them to provide individual, marital, couple, family, and group counseling for a social work orientation, or a Bachelor’s degree and four years’ of social work experience in the field.\
