Behavioral Health Consultant

Access Community Health Centers is a non-profit organization with the mission of improving the health and lives of people we have served since 1982. We provide high quality affordable health care including medical, dental, behavioral health, and pharmacy services based on our local community's needs, addressing financial, cultural and language barriers.

The Access Primary Care Behavioral Health team is well established with over 15 years of robust integration within the medical home. We work within the Primary Care Behavioral Health (PCBH) model as Behavioral Health Consultants to the primary care providers. BHCs provide targeted assessment and intervention to improve the health and functioning of a culturally diverse patient population across the lifespan. Patients are referred to Behavioral Health for all types of behavioral issues, including physical health problems such as coping with chronic illnesses, medication non-adherence, health behavior/lifestyle modification, substance use concerns, as well as more traditional mental or behavioral health problems. The position also includes working in a residency teaching site of the University of Wisconsin-Madison Department of Family Medicine and Community Health. This provides the opportunity of consultation and educational development of family medicine residents and collaboration with faculty. Additionally, we host students in training for future careers within integrated care, and the position includes the opportunity for supervision. Applicants should have experience with or be highly interested in working with diverse and at-risk patient populations. Summary of Behavioral Health Consultant
Under the direct supervision of the Chief Behavioral Health Officer, the Behavioral Health Consultant is responsible for patient care and a variety of administrative tasks. The Behavioral Health Consultant, as part of the primary care treatment team, identifies, triages, and manages patients with medical and behavioral health problems within the primary care setting. In addition, the Behavioral Health Consultant will provide skill training through psycho-education and patient education strategies and will develop specific behavioral change plans for patients and behavioral health protocols for target populations.

Required Qualifications:
  1. Doctoral degree (PhD or PsyD) in Psychology or Clinical Psychology OR Master's degree in Social Work (MSW)
  2. Current Wisconsin licensure as a Licensed Clinical Psychologist or Licensed Clinical Social Worker

Preferred Qualifications:
  1. Prior work with diverse patient population
  2. Bilingual in Spanish and English.
  1. Prior work experience as a Clinical Psychologist or Social Worker in a medical setting
