This is a Full Time, 32 hours a week, 64 hours a pay period position. Shifts are Monday - Friday, no weekends. Typical shifts are 10:00am - 4:30pm (with flexibility).

Scope:

  • Provide superior customer service in all interactions with internal team, referral sources and clients for Home Care.
  • Responsible for daily operations for new referrals and evaluations.
  • Responsible for timely initiation of care.


Key Responsibilities:

  • Provide superior customer service in all interactions with internal team, referral sources and clients, caregivers.
  • Ensure customer service is prioritized in all interactions to ensure a positive, smooth and efficient referral process for clients, internal teams and referral sources. Go above and beyond to ensure our agency is easy to work with.
  • Open communication with supervisor and Regional Managers on any client concerns, clinical complexities, and identify necessary steps to correct any issues/concerns immediately.
  • Responsible for daily operations for managing new referrals and evaluations.
  • Follow documented processes to prepare new referrals and evaluations for handoff while ensuring timely initiation of care.
  • Collaborate with clinical managers to ensure awareness of staffing availability and schedule efficiency for nursing and therapy for all new admissions.
  • Work closely with the Intake Team to receive referrals, contact client in a timely manner, triage their needs, and schedule & communicate appropriately.
  • Work quickly and efficiently to assess new referral needs, appropriate care, and priority.
  • Lead the scheduling of admissions with clinical staff to ensure timely initiation of care.
    Overcome objections from clients or caregivers regarding starting services to improve overall conversion rates..Work with Customer Service Manager on challenging situations that may result in a non-admission due to client/caregiver refusal.
  • Follow up with Account Executives and referral sources regarding non-admissions, when appropriate.
  • Support the intake process as needed.

Education and Experience Required:

  • Ideal candidate would be a Licensed Practical Nurse in the State of Minnesota.
  • Must have proficient computer skills.
  • Must complete and pass a state required background study.
  • Able to communicate verbally and in writing to the extent required by the position.
  • Able to physically perform the duties required by the position.
  • Required to have the ability to read, write and speak English well enough to communicate with staff, residents and visitors.
