Job Summary

The Medical Home Quality Coordinator (RN) is responsible for providing clinical support, coordination and facilitation of quality measures to participating clinics in the medical home program. This position will support new clinics coming onto the program with onsite visits and conference calls for clarification, education and follow-up on any program needs related to the clinical quality measures and program operational details.

This position can be based in the office or have the flexibility of fully telecommuting. 

Essential Functions
  • Builds and maintains strong relationships with clinics who are a part of the medical home program and assists in education and communication of essential program details.
  • Provides support and coaching to participating clinics in meeting specific quality measures by facilitating conference calls and onsite visits.
  • Functions as a resource for the medical home program providing best practices in clinical measures and quality improvement initiatives.
  • Provides education and resources about population health management principles to support primary care clinics.
  • Partners with internal and external stakeholders to understand data needs and delivery insightful solutions.
  • Researches and analyzes clinical data providing key metrics in order to measure performance and guide improvement initiatives.
  • Travel outside of core business location required.

Education Requirements

Required: Associate's

Preferred: Bachelor's

Field(s) of Study: Completion of RN program is required; BSN is preferred.

Experience Requirements

Required: Minimum of 5 Years

Preferred: Minimum of 7 Years

Experience Details: Minimum of 5 years nursing experience is required; ambulatory nursing and patient centered medical home care model experience is preferred; ideal candidate possesses population health management and patient centered medical home care model knowledge.

Certification, Licensures & Registration Requirements

Title: North Dakota (ND) Registered Nurse (RN) License or RN License valid for work in ND

 

State or Agency: Board of Nursing

Title: Must maintain an active and valid/unencumbered driver’s license which allows for standard driving privileges in North Dakota and Minnesota

State or Agency: Department of Motor Vehicles

Equivalent combination of education, experience or training determined to be acceptable by Human Resources may be substituted, unless regulated by contract or program standards

Job Posting Policy

Employees new to Blue Cross Blue Shield of North Dakota are eligible to apply for positions within their assigned department after successfully completing 90-days in their role. For positions outside of their department, new employees should attain a minimum of six months of service before applying for a new role.
