Job Summary
The Pharmacy Clinical Coordinator is responsible for the day-to-day coordination of the varying Pharmacy clinical services, is involved in an active clinical practice, and represents the Pharmacy department and facilitates various hospital committees as assigned. The Pharmacy Clinical Coordinator ensures the rational, safe, and pharmacoeconomic use of drugs within Altru Health System, and participates in assessing, planning, implementing, evaluating and documenting the care of patients. The Pharmacy Clinical Coordinator serves as an advocate of the pharmacy industry.

The Pharmacy Clinical Coordinator ensures the rational, safe, and pharmacoeconomic use of drugs within Altru Health System, and participates in assessing, planning, implementing, evaluating and documenting the care of patients. The Pharmacy Clinical Coordinator serves as an advocate of the pharmacy industry.
Essential Job Functions
- Identifies, conducts follow-up, and takes action on unsatisfactory outcomes, unusual findings, special needs, and requests.
- Administers care to patients, maintaining clinical skills, providing instruction and maintaining continuity of patient care.
- Serves as an educator to the community/region on Pharmacy issues.
- Develops and maintains clinical programs, guidelines, and protocols across disciplines to support the clinical needs and monitoring of our patients, including but not limited to antimicrobial stewardship program management.
- Assists in facilitating, coordinating, and monitoring students' clinical experiences.
- Develops Quality Assurance/Drug Utilization Review and adverse drug reaction reporting programs according to standard operating procedure.
- Assures completion of documentation according to standard operating procedure.
- Functions as a resource to Pharmacy staff and medical staff regarding the appropriateness of drug therapy for patients.
- Performs other duties as assigned or needed to meet the needs of the department/organization.
- Demonstrates understanding and follows infection control policies and procedures according to standard operating procedure.
- Maintains requirements for age specific competencies for the position and demonstrates the knowledge or principles of growth and developments and the skills necessary to provide patient/customer care/services.
- Demonstrates knowledge and understanding of and compliance with:
- All pertinent safety, health and environmental policies, procedures and guidelines.
- Working safely, without causing harm or risk to self, others or property.
- The work environment safety procedures and prompt reporting of unsafe practices, procedures, accidents, injuries or other safety violations to the Leader.
- The use of all safety equipment and personal protective equipment.
