Dear Virginia, we're currently seeking a dynamic, quality driven part-time  Nurse Practitioner  to join our growing medical team, Legacy Medical.  Legacy Medical is a division of Edgewood Healthcare, a company that's been serving and caring for seniors since 1993.

The Nurse Practitioner is responsible for  providing primary care services to patients in congregate living environments such as Assisted Living or Skilled Nursing Facilities, in-home environments and via telemedicine. 

We're looking for someone equally as  dedicated to the delivery of effective, evidenced-based clinical practice provided through our on-site, in-home platform.  Our teams are passionate about helping patients maintain quality of life while providing ease of access to quality medical services. We strive to develop a communicative environment in which each individual feels comforted, cared for and involved in decision making related to their own care.

Responsibilities include but are not limited to:

Provides patient/patient's family or caregiver with appropriate education based on the patient's condition and identifies needs in order to optimize understanding.  Provides health maintenance and disease management counseling to patients.
  • Obtains comprehensive medical histories, performs physical/examinations/assessments
  • Provides medical management and/or other interventions for acute and chronic health problems
  • Implements a plan of care and performs procedures according to the Advanced Practitioner's education, training, experience and competencies.  Refers to specialists, therapists, home health and/or hospice in a timely manner, ensuring continuity of care.
  • Orders and interprets lab studies and diagnostic tests to evaluate the effectiveness of treatment as appropriate.  Notifies patients promptly of test results.
  • Writes orders for prescription medication under the established protocol.
  • Serves as a liaison and resource for healthcare team members, patients and families.


Minimum Requirements

 

The Nurse Practitioner collaborates with physicians, as necessary, to verify diagnosis, identify high-risk situations or alerts to changes in patient's status and takes appropriate action. Administers medication and performs procedures within the scope of practice. This position provides patients/patient's families or caregivers with appropriate education and follow-up care and/or referral to other health care resources.

A successful candidate possesses the following:
  • Master's Degree in Nursing
  • Valid nurse practitioner license
  • Demonstrated current competence and provision of care, treatment, or services for varied volume of patients. 
  • Minimum of 3 years' nursing experience
