Overview

The Inpatient Diabetes Educator is responsible for the education of patients, their families and appropriate support systems regarding diabetes self-management education. The Inpatient Diabetes Educator provides assessment, planning, implementation and evaluation related to diabetes care.

Responsibilities/Job Description

General Patient Care Principles

* Respects confidentiality of all information by sharing that information with those who need to know or have attained proper permission.
* Meets all role-related and age-specific competencies.
* Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served.

Patient Education

* Performs learning needs assessment including health literacy.
* Assess the patient's knowledge of diabetes and self-care skills.
* Initiates education for the following patients:
* Newly diagnosed with diabetes/hyperglycemia
* When diabetes complicating factors arise
* Uncontrolled hyperglycemia and/or knowledge gaps

* Evaluates the patients comprehension of completed education and develops follow up plans to address ongoing needs.
* Documentation and communication of required assessments, interventions, evaluations, and follow-up is consistent with established departmental and national standards.
* Individualizes education to the patient and patient circumstance.
* Utilizes program content which is consistent with national standards and M Health/Fairview Diabetes Care.

Care Coordination and Transitions of Care

* Utilizes a patient and family-centered approach.
* Collaborates with other team members.
* Empowers patients and caregivers to actively participate in their care.
* Provides a clear and feasible management plan at discharge based on clinical needs, patient preference, goals, and abilities.
* Evaluates the ability of the patient to obtain diabetes supplies and medication.

* Ensures that a follow-up appointment for diabetes care is scheduled prior to discharge.
* Obtains referrals to community resources to continue diabetes self-management education.

* Communicates the status of diabetes self-management education, plan of care to the next provider and/or site of care.
* Identifies and mitigates risk factors for readmission.

Collaboration

* Actively participates and/or communicates with the system inpatient diabetes steering committee to ensure consistency in curriculum, tools and data collection.
* Utilizes the support systems and tools to provide services and make recommendations for changes as needed.
* Encourages and supports the quality clinical initiatives related to diabetes care.
* Aware of the quality measures and monitors that are in place.

Professional Practice

* Demonstrates accountability for professional practice by accepting responsibility for assessment of performance, peer review, and by engaging in ongoing learning experiences.
* Practice is evidenced-based, in addition to the Scope and Standards of Practice for Diabetes Educators as outlined by the American Association for Diabetes Educators; Minnesota standards guidelines, and standards for M Health/

Qualifications

Required

Education

* Bachelor's Degree

Experience

* 3-5 years' experience in diabetes education

License/Certification/Registration

* Registered Nurse (RN) or Registered Dietitian (RD).
* Certified Diabetes Educator (CDE) required upon hire or obtained within one year of hire.
