Job Summary

The Patient Advocacy Coordinator plans, directs, and coordinates activities for providing community care services for patients and/or guarantor(s) by developing collaborative working relationships with Altru Health System (AHS) staff and community agencies. This role also works to identify uninsured patients and reviews for appropriateness of referral. The Patient Advocacy coordinator audits and initiates handoffs of Community Care applications. The coordinator works collaboratively with patient care staff to assist patients with their financial needs and assists in creating short and long-range objectives.
Essential Job Functions
  • Plans, directs and coordinates the activities for providing community care services for patients and or guarantor(s).
  • Develops, implements, and coordinates financial assistance programs. Works collaboratively with patient care staff to assist patients with their financial needs.
  • Gathers information from patients to determine their eligibility for Altru Community Care and assists patients to complete the application.
  • Identifies uninsured patents and reviews for appropriateness of referral to community agencies/programs including our Altru Community Care Program.
  • Audits and initiates handoff of Community Care applications for available programs in other Altru Health System areas.
  • Develops a collaborative working relationship with Altru Health System (AHS) staff and community agencies, i.e., CSS, SSA, and Third Street Clinic.
  • Coordinates coverage for all mini Business Office locations. Provides training and education for all mini Business Office staff.
  • Performs other duties as assigned or needed to meet the needs of the department/organization.
  • Demonstrates understanding and follows infection control policies and procedures according to standard operating procedure.
  • Maintains requirements for age specific competencies for the position and demonstrates the knowledge or principles of growth and developments and the skills necessary to provide patient/customer care/services.
  • Demonstrates knowledge and understanding of and compliance with:
    • All pertinent safety, health and environmental policies, procedures and guidelines.
    • Working safely, without causing harm or risk to self, others or property.
    • The work environment safety procedures and prompt reporting of unsafe practices, procedures, accidents, injuries or other safety violations to the Leader.
    • The use of all safety equipment and personal protective equipment.

  • Frequent Drivers: Persons whose main responsibility is not driving, but drive daily or almost daily.
