Thomas Allen's Case Management Division is Hiring!

LOCATION: Office in West St. Paul, MN with frequent travel for visits with individuals served in the Twin Cities Metro.

This position is in-person. Hybrid work-from-home opportunities are available after the first 6 months of employment!

HOURS AVAILABLE: Flexible Monday - Friday 8am - 4:30pm

JOB SUMMARY: As a Case Manager, you will conduct assessments, create and review care plans, and coordinate services with individuals served and the interdisciplinary team. You will also connect individuals served with community resources and ensure waiver coordination and documents meet lead agency guidelines.

REQUIREMENTS/MUST HAVE:

* Bachelor's degree in social work and LSW, or a bachelor's in a related field such as psychology, human services, or sociology
* Must have at least one year of full-time experience working with DD
* Background clearance
* Ability to effectively communicate in English, written and verbally
* Driver's license, reliable vehicle you can use for work purposes, acceptable driving record, and proof of auto insurance required
* Must be at least 18 years of age

NICE TO HAVE:

* Experience with Rule 185 and DD Waiver
* Knowledge of MA, Medicare, and MN Healthcare programs
* Knowledge of community resources and providers
* Great organizational skills and attention to detail
* Knowledge of computer basics and familiarity with Microsoft Office suite
