Rehabilitation Specialist
Community Care is hiring a Rehabilitation Specialist to join our Program team. This rewarding opportunity is perfect for someone who enjoys team collaboration, community case management, being part of a mission driven organization and who shares our mission to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This is a full-time opportunity at our Racine facility.
Rehabilitation Specialist Responsibilities:
* Consults on members' need for adaptive and/or durable medical equipment (DME) and/or home modifications.
* Serves as a resource to teams in care planning, problem-solving and providing resources and interventions for members. Will develop and present training to teams including assisting with new employee orientation.
Rehabilitation Specialist Requirements:
* Education: Bachelor's degree in Occupational Therapy or Physical Therapy is required. Masters degree in Occupational Therapy or Physical Therapy preferred. Doctorate in Physical Therapy preferred
* Experience: A minimum of 3 years recent clinical experience with adults over 65, or persons with physical or developmental disability in a hospital, home care, or nursing home setting required. Preferred: Strong background in home care, case management, or other aspect pf multidisciplinary care management; deep understanding of DME and seating/mobility products.

* Monday to Friday
* 8:00AM - 4:30PM
