Overview:
Fairview Health Services is looking for a Transplant Coordinator to work out of the University of Minnesota. This position is approved for 64 hours every two weeks and is benefit eligable. This position will be working in Post Transplant Heart.
This position requires remote work. In addition to adhering to all Fairview policies, guidelines, procedures, the Fairview Commitments, and the Code of Conduct, it is the expectation the following requirements of the remote work policy will be met, including but not limited to:
Remote workspace that is clean, safe, and professional.
Maintains an internet connection consistent with Fairview standards at remote workspace.
Remain productive and responsive during their scheduled work hours.
Attend primary Fairview location, as needed
Responsibilities/Job Description:
The Transplant Coordinator is responsible for coordination and preparation of patients for organ transplantation and the long-term management of patients after transplant through collaboration with various members of the interdisciplinary team.
Coordinate patient care in both pre- and post-transplant phases
Collaborate with interdisciplinary team members within the organization and outside community partners
Act as a liaison for transplant physicians to implement medical management of conditions
Provide telephone triage and make patient care decisions based on patient symptom assessment and evaluation, laboratory results, and patient condition changes
Apply appropriate medical interventions and transplant protocols based on patient assessment, laboratory results, and patient condition changes
Assumes on-call responsibilities after office hours by providing a resource for medical triage. Acts as transplant coordinator when donor organ becomes available.
Provides on-going education to patients and families
Collaborate with other members of the clinical team to improve patient care and benchmarks through quality improvement projects
Participate in the development and revision of protocols
Maintain knowledge of current practices in transplantation, nursing, and healthcare
Qualifications:
Required
Education
BA/BS in Nursing
Experience
3-5 years acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg
License/Certification/Registration
MN RN license
