We're in search of a Registered Nurse (RN) to join our  CaringEdge team ! CaringEdge is the health services division of Edgewood Healthcare, a company that's been serving and caring for seniors since 1993.

Responsibilities:  

The Home Health RN is involved in planning, organizing, and directing home health services. This includes the coordinating of community resources to plan and direct services to meet the needs of individuals and families within their homes and communities.   
  • Comprehensive assessment of patient and family to determine home care needs. 
  • Provides a complete physical assessment and history of current and past illness. 
  • Coordinates appropriate care planning with patient and family. 
  • Continuously works toward discharge planning needs and goals with patient, family, and interdisciplinary team. 
  • Effectively communicates patient and family needs to interdisciplinary team. 


Minimum Requirements

 

You would be a great addition to our team if you possess the following:
  • Graduate of accredited school of nursing 
  • Registered Nurse with current licensure to practice professional nursing in the state. 
  • Three years of experience in a home health preferred but not required. 
  • Must be a licensed driver with an automobile that is insured in accordance with state/ or organization requirements and is in good working order 
  • Excellent observation, verbal and written communication skills. 
  • CPR certification 
  • Participation in on-call duties
